ACCOUNTABILITY AND COMMUNITY SAFETY (ACS) FORMS

CONFIDENTIAL INCIDENT INTAKE FORM

We are creating a confidential database to help people report incidents of harassment, abuse, discrimination and assault and aid them in taking action if they desire to do so. Those who are survivors and/or have been victims of Sexual, Gendered, Racial, Ableist, and other forms of harm may use this form to document incidents they have experienced. We hope this tool will help us identify serial perpetrators of harm and build paths towards community accountability.

SUPPORT GROUP FORM

Our ACS Committee is holding space for victims/survivors of harm in the music community. To take part, fill out this form or share it with your communities.
